Note: We receive many anguished letters for help, but many times people are so angry and have been involved with the case for so long that they forget to tell us the whole story, from the beginning. But it is very important to lay out the facts, in order. This form is intended to help you do that. provide us with as much information as you can.

Most of the boxes in this form will expand to accomodate as much information as you wish to give us.
You don’t have to use this form. You might prefer to print it out and use it as a guide to answer the questions in a word-processing program such as Microsoft Word or Wordperfect. You would then email the file to help@ncrj.org.

1. Narrative. This part is optional. Answering questions 2 through 17 should give us the information we need. But many people like to begin by just telling the story, as if they were telling a concerned stranger what happened. If you provide this narrative, there is no need to be formal or legalistic. Start at the beginning with the background; explain how the allegations arose; continue with what happened.
